Is it permissible to assist a well-off friend who is about to undergo surgery with money designated for poor students?

It is not permissible to dispose of Zakat that has been entrusted for distribution to specific students by giving it to others, even if they are deserving, let alone those who are not, because the agent is bound by what his principal has authorized him to do.
