And does his divorce occur if the obsessive thoughts compel him to it, and he says to himself, "I want to divorce her"?

It is not a prerequisite for the divorce of the one afflicted by waswasah (obsessive doubts) not to have thought about divorce beforehand. The decisive factor is his state at the time of uttering it. If it was due to waswasah, the divorce does not occur.
