The answer

The intention for performing Hajj on behalf of one's father is to say: "Here I am, O Allah, for Hajj on behalf of my father" if you are performing Hajj mufrad (Hajj only), or "Here I am, O Allah, for Umrah, followed by Hajj, on behalf of my father" if you are performing Hajj mutamatti' (combining Umrah and Hajj with a break in between). Then, on the Day of Tarwiyah (8th Dhul-Hijjah), you make the intention for Hajj and say: "Here I am, O Allah, for Hajj on behalf of my father," or "Here I am, O Allah, for Umrah and Hajj on behalf of my father" if you are performing Hajj qiran (combining Umrah and Hajj without a break). This is supported by the Hadith of the man who said: "Here I am for Shubrumah," and the Prophet, peace and blessings be upon him, said to him: "Perform Hajj for yourself first, then perform Hajj for Shubrumah." This intention suffices for all the rituals.
