Is a woman considered irrevocably divorced (three times) after her husband repeated the pronouncement of divorce twice the first time, and then divorced her a third time two months later, knowing that she had returned to him after the first fatwa?

If a husband says to his wife: "You are divorced, divorced," then a single divorce occurs, unless the husband intended to effectuate two divorces, due to the absence of anything that necessitates a distinction. So, if your husband did not intend to effectuate two divorces, then it is a single divorce, and what Dar Al-Ifta (House of Fatwa) ruled regarding the permissibility of your husband taking you back is correct. The husband must be careful about uttering divorce, because he has only one divorce left, after which she will not be lawful for him unless she marries another husband and he consummates the marriage with her, and then he divorces her, as stated in verse 230 of Surah Al-Baqarah.
