The breaking of an oath is not repeated with the repetition of the act unless there is an intention or wording indicating it. The aforementioned wording does not imply repetition. Therefore, the expiation is not repeated with the hitting unless the intention of repetition was present when the oath was sworn. It should be noted that hitting the sister is not permissible because it is an assault, and she is not held accountable by Islamic law for her actions due to her insanity.
