The preponderant opinion is that congregational prayer is obligatory unless there is an excuse, and among the excuses for not performing it is illness. So, if the disability prevents going to the mosque or causes hardship, or if there is no one to push the wheelchair except with difficulty, then you are not obliged to pray in the mosque. Pray at home, you and your family, and you will be rewarded for your intention.
