What is the ruling on working in a health club that belongs to a hotel which serves alcohol in an Arab country?

If your work in the health club involves serving alcoholic beverages or other forbidden things, such as supervising mixed-gender swimming pools, then such work is not permissible for you, due to the Almighty's saying: "And do not cooperate in sin and aggression." However, if your work is not related to serving alcoholic beverages and you do not attend this forbidden act, then there is no harm in it. It is preferable to seek work outside of hotels where unlawful acts are practiced.
