If a fasting person swallows medicine pills that pass the throat and reach the esophagus before the dawn adhan, their fast is not broken by the medicine reaching the stomach after the adhan, because the act of swallowing occurred before dawn, and the esophagus is the place of swallowing. Their hesitation about breaking the fast does not invalidate it.
