Is it considered fornication for a husband to rub his penis against the clitoris and anus, then insert its tip and ejaculate inside the vagina during menstruation?

It is permissible for a man to enjoy the exterior of his wife's anus without penetration, and this is permissible with no sin in it. As for fondling the exterior of the vulva (vagina) without a barrier during menstruation, it is forbidden according to most scholars, and inserting the tip of the organ into it is forbidden by consensus. Therefore, it is not permissible to allow the husband to do that.
