What is the ruling regarding a woman who accidentally caused the death of her child by ingesting kerosene: is she required to fast for sixty days or offer expiation?

If the mother placed the kerosene in its usual place, out of the reach of children, and the child drank from it and died, then there is no blame on her. However, if she placed it in an unusual place, enabling her children to reach it, and one of them drank from it and died, then this is considered accidental killing, for which blood money is due from the killer's 'asabah (agnatic relatives), and expiation is due from the mother, which is the freeing of a slave. If she cannot find one, then it is fasting for two consecutive months. Children must be taken care of and kept away from what harms them. It is not permissible for a person to consume something harmful for experimentation, as Allah Almighty says: (And do not throw [yourselves] with your [own] hands into destruction).
