What is the situation in which a person suffering from obsessive-compulsive disorder (waswās) is considered reassured concerning his divorce for it to take effect, and is it permissible for him to adopt the fatwa that a triple divorce does not count as a single divorce?

Divorce issued by one afflicted with obsessive-compulsive disorder (waswas) without their conscious choice is not effective. However, divorce issued while one is aware and acting out of their own choice is effective.
