Fasting Ramadan is obligatory for the able Muslim. Breaking the fast is permissible for the sick person who would be harmed by fasting or who needs treatment during the day. If it is possible to forgo the medication without harm or hardship, then fasting is obligatory. If the medication is necessary and abstaining from it would cause harm, then it can be taken at the beginning of the night. If it must be taken before sleep and causes severe hunger and thirst, then one should intend to fast and continue fasting until harm is feared. At that point, one may break the fast only to the extent necessary to alleviate the harm, then resume abstaining for the rest of the day and make up the fast later. If no harm is feared, then breaking the fast is not permissible, similar to the case of those in physically demanding professions.
