Is it a condition for perpetual charity (Sadaqah Jariyah) to be performed by the deceased himself before his death, or is it permissible for his family to give it on his behalf after his passing? And does the reward for it reach the deceased if he refrained from giving charity during his lifetime despite possessing wealth?

A جاری صدقہ (perpetual charity) is given from the owner's wealth during their lifetime, like building a mosque, or after their death, whether performed by a relative or a stranger. If the mature heirs give charity from the inheritance on behalf of the deceased, or build a mosque for them, its reward reaches them by the consensus of scholars. Disagreement among them, however, has occurred regarding the arrival of the reward for reciting the Quran, performing prayers, and similar acts.
