Is it permissible to make the intention to perform a two-rak'ah prayer of gratitude, or to fast a certain number of days, should a supplication be answered or a specific matter be realized?

There is no harm for one who intends an act of worship contingent upon a condition, because intention alone is not considered a vow that must be fulfilled. A vow is only binding through verbal pronouncement.
