How can the zakat payer identify those entitled to zakat from among the poor and the needy, and is it permissible to authorize a trustworthy entity, such as Al-Azhar, to deliver it to them?

The eligibility for Zakat is known through several methods: personal knowledge of the poor person's situation, or through trustworthy individuals informing about it, or by the needy person himself informing about his situation, unless his claim contradicts the apparent circumstances. As for delegation in paying Zakat, it is recommended, as it is closer to sincerity and avoids ostentation. It may even become obligatory for those who fear ostentation or are ignorant of its rulings and beneficiaries. Therefore, Zakat can be paid directly to the eligible recipient or through a trustworthy and upright agent.
