How can one get rid of evil thoughts that offend the Divine Self, which affect prayer and reading the Qur'an, along with intense fear of death and the Day of Resurrection? And what is the extent of the sin resulting from them, and how can one atone for them?

There is no accountability for detestable thoughts and whispers (waswas); because they are a sign of pure faith. Indeed, some Companions asked the Prophet, peace and blessings be upon him, about the presence of disbelief-inducing whispers within them that they could not bear to speak of. He replied that this was pure faith. Thus, disliking and rejecting these whispers is a sign of sincere faith and intense fear of Allah Almighty.

Whoever is afflicted with such whispers should turn to Allah with supplication, frequently remember Him, and divert themselves from these whispers, paying them no attention.
