Is a person afflicted with "flat effect"—a condition that causes them to lose the ability to feel emotions toward others, even their parents, and is untreatable—considered religiously accountable and thus obligated to perform prayer, fasting, and zakat?

Flat affect is a symptom, not a disease in itself, and it has a wide range, appearing in personality disorders and psychotic disorders, the symptoms of which can be controlled with treatment. The one afflicted with these disorders is religiously accountable as long as he is sane and aware, for sanity is the basis of accountability, not emotions, as the Prophet, peace and blessings be upon him, said: "The pen is lifted from three: from the sleeper until he awakens, from the child until he grows up, and from the insane person until he regains his sanity." Thus, a mentally ill person who is sane is obligated to pray and fast. If he loses his sanity for some time and then recovers, he must pray the current time's prayer and make up for what he missed. Zakat is obligatory upon the child and the insane person according to the majority of jurists.
