Does a triple divorce take effect if the husband says to his wife, "You are divorced, divorced, divorced," after he had threatened her with divorce twice, and then she requested it?

If the husband intended, by the aforementioned wording, to effect three divorces, then all of them have occurred, and the major irrevocability (al-baynunah al-kubra) has taken place. Thus, you are not permissible for him unless after another marriage with its conditions. However, if he did not intend three divorces, then only one divorce has occurred, and he may take you back during the waiting period (iddah).
