Is a doctor son's refusal to treat his parents and fulfill their doctors' requests considered disobedience, and what is its recompense in this world and the hereafter?

If a son is able to treat his parents [medically] but does not do so, then he is disobedient to them (aqq), and deserves punishment. This is because the rights of parents are great, and Allah has enjoined kindness towards them and forbidden even the word "uff" in their regard. Leaving one's parents to suffer from illnesses while being able to treat them contradicts kindness. It has been reported in the Hadith that Paradise lies under the feet of mothers. So, let the son hasten to be dutiful to his parents and beware of the punishment for disobedience.
