Does a triple divorce take effect if it is conditional on an action that did not actually occur, such as the husband saying to his wife: "If you have spoken to them, then you are triply divorced," when in fact she has not spoken to them?

If you did not speak to your father and brothers, then your divorce has not occurred. Tell your husband the truth, and that the divorce is not binding upon him because he made it conditional on something that did not happen, as stated in Sheikh Ad-Dirdir's "Ash-Sharh al-Kabeer": "You are divorced if you entered the house. If she said, 'I did not enter,' then nothing is binding upon him, unless the contrary becomes evident."
