Is it permissible to pray Istikharah to choose between two matters, such as choosing between two girls who are equal in every respect for marriage to one of them? And if it is permissible, what should be said in its supplication?

There is no objection to performing for multiple matters, such as deciding to marry one of two women. The method is to pray two rak'ahs and recite the Istikhara supplication mentioned for each matter separately.
