How can intention be located in the heart, and is it a present thought or something else?

Intention (niyyah) is the resolve to perform an action. For the worshiper, it is sufficient to have the resolve in his heart to perform the prayer he is about to offer, such as intending to begin the Dhuhr or Asr prayer.
