How can one get rid of and distance oneself from this sin, and is there a prescribed (hadd) or discretionary (ta'zir) punishment for what the questioner has committed, or is repentance sufficient?

Whoever commits these abominations must repent sincerely and immediately, sever all ties with the programs and individuals that lead to them, and adhere to the law of Allah. Among the things that help in this are: realizing the heinousness of these acts and that they alter human nature, remembering Allah’s knowledge and comprehensiveness, frequently contemplating death and what comes after it, striving to guard one’s thoughts, supplicating, accompanying righteous people, and adhering to gatherings of remembrance and knowledge.
