Why did you obligate one who entered prayer without reflection to repeat it in Fatwa No. 132456, even though you stated in Fatwa No. 94062 that intention follows knowledge, and that whoever knows what he wants to do has intended it?

Intention (niyyah) is a condition for the validity of prayer. It must coincide with the takbīrat al-iḥrām (the opening Allahu Akbar) or precede it by a very short time, because the intention usually does not vanish within a short time, unlike a long time. Therefore, whoever intends to pray at home before going to the mosque, their prayer is not valid.
