If it is proven by experience or a doctor's statement that fasting causes you an unbearable hardship due to illness, then you are permitted to break your fast, based on the Almighty's saying: "But whoever among you is ill or on a journey [during them] – then [he must make up] an equal number of other days."
If the illness is chronic and incurable, then feed one poor person for each day. If it is an illness from which recovery is expected, then you must make up the missed days when you are able to do so. You should not switch from making up the days to feeding the poor as long as you are able to make up the days.
