If the health insurance is cooperative (ta'awuni) and permissible, and you acquired the medicine without deception, then it is permissible to exchange it for the device, provided that not using the medicine does not result in significant harm. However, if the insurance is impermissible (haram (definition)), then it is not permissible to benefit from the medicine except to the extent of the premiums paid, and the remainder must be spent on the welfare of the Muslims.
