The answer

Commercial insurance, in all its forms, is Islamically forbidden due to the elements of (excessive uncertainty), jahalah (ignorance regarding the subject matter), and riba (usury), and this is what the Fiqh Councils have unanimously agreed upon. As for cooperative insurance, it is permissible because it is based on donation, mutual support, and cooperation in righteousness, not on profit, though it is rarely found. Accordingly, one must examine the status of the insurance company: if it is commercial, it is not permissible to deal with it; if it is cooperative, then there is no objection.
