Is it obligatory for someone with weak eyesight who has a private driver to order him to take him to the mosque for congregational prayers, or is it permissible for him to pray at home?

Congregational prayer is obligatory for every Muslim who hears the adhan (definition) (call to prayer) and is able to perform it. This is indicated by the Prophet's (peace and blessings be upon him) response to the blind Ibn Umm Maktum, when he sought permission to pray at home. The Prophet asked him: "Do you hear the call to prayer?" He replied: "Yes." The Prophet said: "Then respond." In another narration: "I find no concession for you." Thus, if a blind person or one with poor eyesight finds someone to lead him to the mosque, he is obligated to pray in congregation there, especially if the mosque is adjacent to his home.
