What is the ruling on two adhans and two iqamas when combining two prayers?

There is one adhan (definition) (call to prayer) for the two combined prayers, and an iqamah (definition) (second call to prayer immediately before standing for prayer) for each prayer. This is the view of the Hanafis and Hanbalis, the relied-upon opinion among the Shafi'is, and the view of some Malikis. The evidence for this is the action of the Prophet, peace and blessings be upon him, during the Farewell Pilgrimage, where he prayed Dhuhr and Asr in Arafat with one adhan and two iqamahs, and similarly Maghrib and Isha in Muzdalifah. The ruling for adhan and iqamah is fard kifayah (a collective obligation), and the adhan of the town suffices for its residents, who then perform an iqamah for each obligatory prayer.
