The emission of semen, whether in sleep or while awake, if accompanied by pleasure, necessitates (ritual bath). If there is a legitimate excuse preventing ghusl due to a medical device that cannot be removed, then it is obligatory to wash what can be washed and perform (dry ablution) for the remainder, or perform tayammum fully if ghusl is entirely impossible.
