What you mentioned about water used for ' (washing after defecation or urination) coming out does not nullify wudu', because the nullifiers are limited and well-known. However, if the water used for istinja' entered the body through the anus and then came out, it nullifies wudu' according to some scholars. One should avoid intrusive thoughts (waswasa) and not pay attention to them.
