is obligatory on the money collected for your son's treatment if it reaches the (minimum threshold) and a hawl (full lunar year) has passed over it. As for the bonus, Zakat is not obligatory on it until a hawl passes over it. So, if a hawl passes while it is nisab on its own, or by combining it with other money over which a hawl has passed, then Zakat becomes obligatory on it.
