The question

How does a person afflicted with obsessive-compulsive disorder, hemorrhoids, and a fistula—from which purulent and bloody secretions emerge in varying quantities, necessitating the use of diapers—purify himself according to the Maliki school of thought? And when? And what should be done regarding prayer and touching the Qur'an, given the psychological and material hardship of washing the anus and changing the diaper before every prayer?

The answer

The Maliki school of thought is lenient in these matters. All you need to do is perform ablution (Wudu') after an optional impurity (hadath ikhtiyari), and then you do not need to repeat the ablution even if the time for prayer expires, unless you intentionally incur another impurity. There is no obligation to guard against anything that exits.
