Is a documented divorce considered the third, when it is in fact the second, and the husband neither uttered nor intended the divorce?

If a person makes a mistake in what they tell the ma'dhun (authorized marriage official) because their tongue slipped, then there is no consequence for them, neither in religious rulings (fatwa (definition)) nor in judicial rulings (qada'), if it is proven that it was merely a slip of the tongue. However, if they intended to lie in their statement and did not intend to initiate a divorce, it has been said that the divorce becomes binding both outwardly and inwardly. Another opinion is that it becomes binding outwardly only.
