If your husband made your divorce conditional on his return to adultery, and he did return to it, then that is the second divorce. However, the third divorce does not take effect as long as the husband denies committing adultery, for in matters of divorce, the husband's word is taken, and the default is the continuation of the marriage; thus, divorce is not proven by doubt. It is not possible to issue a ruling on whether the wife is forbidden to her husband or not until it is confirmed that he has divorced her three times.
