You must immediately distance yourself from her because she is a stranger to you after the triple divorce, and intimacy between you is forbidden. The conditional divorce you mentioned has no effect because it occurred after the termination of the marital bond, so it does not apply to a valid subject. According to the majority of scholars, this woman is irrevocably divorced from you with a major separation, so her conditional divorce holds no weight. If, by conditioning the divorce, you only intended to threaten her and not actually divorce her, then you only owe an expiation for breaking an oath if you violate it. This violation occurs if her inability to go with you becomes permanent, provided you did not intend a specific time frame.
