Does adherence to praying for the Prophet with the form "O Allah, bless and grant peace upon our Prophet Muhammad" fall under the meaning of the hadith "Then your worries will be sufficient for you, and your sins will be forgiven"?

The best wording is the Ibrahimiyah prayer (Salat al-Ibrahimiyah), which the Prophet (peace be upon him) taught to his Companions, and it is: "O Allah, bless Muhammad and the family of Muhammad, as You blessed the family of Abraham. Indeed, You are Praiseworthy and Glorious. O Allah, grant favors to Muhammad and the family of Muhammad, as You granted favors to the family of Abraham. Indeed, You are Praiseworthy and Glorious." The fact that the Prophet (peace be upon him) taught this specific form to his Companions is used as evidence that it is the best way to invoke blessings upon him.
