What are the many mistakes that occur in supplication and prevent its acceptance?

Errors in supplication are numerous, most of which fall under transgression. These include: associating partners with Allah, innovative intercession (tawassul), wishing for death when afflicted, supplicating for the hastening of punishment, supplicating against family and wealth, supplicating for the severance of kinship ties, restricting mercy, showing disrespect towards Allah, relying on others for supplication, despair and lack of certainty, excessive raising of the voice, saying "O Allah, I do not ask You to avert the decree, but I ask You for gentleness in it," and excessive prolongation of the qunut in calamities in an inappropriate manner.
