What is the ruling on one who said to his wife: "You are divorced and forbidden to me with the حرمة (sacred inviolability) of a mother and a sister," intending that, and this was the second divorce, and how can she be returned to his عصمة (matrimonial bond)?

What transpired from the man is a divorce followed by ẓihār. If the wife was madkhūl bihā (had been consummated with) and the ẓihār occurred during her ʿiddah (waiting period), then in order to take her back, he must pay the kaffārat al-ẓihār (expiation for ẓihār) mentioned in the Holy Qur’an: freeing a slave. If he does not find one, then fasting for two consecutive months. If he is unable to do so, then feeding sixty poor persons. This expiation is obligatory in this specific order for anyone who wishes the marriage to continue.
