Is it permissible to give charity to a hospital that treats the poor for free and is managed by an owner around whom suspicions are raised, given the presence of a board to monitor the management, and the uncertainty that 100% of the donations will reach the patients?

If the hospital benefits the poor and the needy and is not supported by the state, then there is no harm in donating to it. It is advisable to strive to prevent the official from gaining control over the funds through good oversight or social pressure. If the benefit of supporting the hospital is strong and its utility is general, and financial corruption is little compared to the achieved benefits, then there is no harm in donating. In-kind donations of medicines and equipment are preferred if they reduce corruption. And whoever wishes to be cautious, let him give charity himself to ensure that the charity reaches the poor.

Sheikh Ibn Uthaymeen was asked about paying Zakat to a charitable organization, and he replied that it is permissible if those in charge of it are trustworthy, and it is preferable to inform them that it is Zakat. It is better for the one giving charity to deliver it himself to be assured that it reaches those who are entitled to it.
