How sound is the Hadith of Musa ibn Talhah regarding, "Zakat is not to be taken except from four things: wheat, barley, dates, and raisins," and is it to be acted upon if it is sound?

The aforementioned hadith is weak and not authentic. A narration from Imam Ahmad states that zakat on grains is obligatory only for the four types mentioned in the hadith, but this narration is based on the weak hadith, and this hadith is not acted upon.
