The sick person must perform ablution (wudu') unless the doctor informs her that water would harm her. If she is unable to perform it herself, a female relative or another person should help her perform it. If no one is available except for a fee, she must pay it if she is able. If the doctor informs her that using water would harm her, then (dry ablution) is permissible for her. If she is unable to perform tayammum herself, someone else should perform it on her behalf.
