What is the ruling on a bank waiving the debt of a deceased employee who has young children?

If the loan is interest-based (usurious), it is forbidden. If it is a good loan (qard hasan), it is permissible. If the bank is Islamic and waives the loan for the employee, there is no harm in that. However, if the bank is interest-based (usurious), the preponderant opinion is that it is permissible, though disliked (makruh), to accept its waiving of the loan, based on the principle that waiving a debt for the debtor is a transfer of ownership that requires acceptance. Similarly, it is permissible, though disliked, to accept a gift from someone whose wealth is mixed, meaning their unlawful wealth is mixed with lawful wealth.
