Does a triple divorce occur if a woman says to her husband: "By triple divorce – there is no going out of the house again while I am not present," and then she leaves to buy medicine while her husband is traveling, and is this divorce considered an irrevocable divorce?

If a husband conditions his wife's triple divorce upon her leaving the house in his absence, then as soon as she leaves, the triple divorce takes effect according to the majority of scholars. However, Sheikh al-Islam Ibn Taymiyyah holds that an expiation for an oath (kaffarat yamin) is required if the husband did not intend divorce but rather a threat, and if he did intend divorce, then only a single divorce takes effect. According to the view of the majority, which is the more preponderant opinion, the woman becomes unlawful for her husband until she marries another husband in a valid marriage and he then divorces her after consummation. An exception to this is if the husband's intention was to specify her leaving for a particular reason, or if the cause that led him to make the oath ceases to exist.
