What is the religious ruling regarding a wife remaining with her husband who betrays her, prevents her from seeing her family, mistreats her and them, does not pray, and doubts her, given the husband's prior knowledge that the wife was not a virgin?

If the husband does not pray, he is a bad man, and it is recommended to separate from him. The wife should advise him to be guided or delegate someone to advise him. If he repents and becomes righteous, then that is good and blessed. Otherwise, it is recommended for her to seek a khula (divorce initiated by the wife). A woman is not obligated to inform a suitor about the loss of her virginity unless he stipulates it. If the reason was committing an immoral act, she should not inform him of the reason but rather conceal it and use allusive language.
