Does a triple divorce occur through the words "You are divorced by three" said by the husband to his wife while he was angry and did not intend divorce, and is it considered a single divorce if it occurs?

Divorce does not occur when one is in a state of uncontrolled anger, nor does it occur due to a slip of the tongue without intending the words. This is the view of the majority of scholars. However, if the words were intended and what was said was understood, then divorce does occur.
