If the hospital pays a known commission to those who bring patients, this is a permissible brokerage and not a bribe, as long as it is free from deception and fraud. The husband's work, as described, is not blameworthy.

Even if the hospital is forced to pay the commission to avert the evil of paramedics, this commission is forbidden for the paramedics. However, it seems that this does not make the husband's work and earnings forbidden, as long as his work involves no deception or falsehood.

And if it is assumed that there is a doubt concerning the husband's money, then money that is forbidden for the one who earns it does not become forbidden for others who receive it rightfully.
