How can the rulings of "the obligation to wash urine if it goes beyond the exit point" and "no pardon for much or little menstrual blood" be reconciled with the leniency of Islam, knowing that I suffer from obsessive-compulsive disorder when performing istinja' (washing after defecation or urination) during menstruation due to the spread of impurity from water splashes mixed with blood, which drives me to take special measures to verify purity, so is this considered extremism?
We ask Allah Almighty to relieve the questioner's distress and remove the hardship she is suffering due to whispers (waswas). We advise her to fear Allah and to make things easier for herself and abandon these whispers, for her succumbing to them has led her to this state. The water separated from the two exits (front and back private parts) is considered pure unless it changes with the color of blood or anything else. There is no need for excessive effort in washing the menstrual exit and removing blood before it stops; rather, it is sufficient to wipe or wash around the pad and leave the exact spot, completing the rest of the ' (cleansing after excretion). If she does that, she should not pay attention to whispers of impurities spreading throughout the house, for the default state is purity until certainty of the opposite is established. It must be taken into consideration that a menstruating woman is not required to purify herself for prayer, and the obligation to wash off impurities that have spread beyond the exit and the non-excusability of menstrual blood apply during purity, not during menstruation.
