What is the appropriate formula for dedicating the recitation of the Holy Quran to the soul of the deceased?

Reciting the Qur'an and dedicating its reward to the deceased is permissible, according to the preponderant opinion. There is no specific formula for this mentioned in the Sharia. So, if the reciter says after finishing the recitation, "O Allah, if You have accepted this recitation of mine, then make its reward for so-and-so," it is good.
