Is a Muslim obligated to be confident in God's answer to his supplication, even if he suffers from a health problem such as low sperm count, and does not desire medical treatment or Ruqyah, and is content with supplicating once and trusting in the answer? And does this include the supplication of a disobedient Muslim who consumes unlawful earnings?

Certainty is a condition for the acceptance of supplication, as the Messenger of Allah, peace and blessings be upon him, said: "Supplicate to Allah while you are certain of the answer." So, a Muslim supplicates with certainty and insistence, knowing that supplication is an act of worship. The answer may come in the form of fulfilling the request, averting harm, or storing it for the Hereafter. The sinful and disobedient person may have their supplication answered, for Allah answers the distressed, even if they are disbelievers. However, the answer to the supplication of a non-Muslim or a sinner depends on Allah's will, and it may be a trial. The Muslim is commanded to supplicate and to abstain from sins and repent, because consuming unlawful things and committing sins are among the impediments to the acceptance of supplication.
